Include the length of time you expect to have the field per day (ex: 2.5hrs)

RULES: No alcoholic beverages, drugs, or unleashed pets on the ECEC recreation field. Smoking in designated areas only. ECEC does not provide first aid or medical supplies for the field. Must be provided by user group. For emergencies call 911 and your ECEC contact (TBD during check-in). All facilities close at dusk, unless lighted. Field use by permit only. No one but members of said group should be allowed use under this permit. Additional portable toilet rental fee may be required. Outside food vendors, including food trucks, must be cleared 2 weeks prior to the event. No trash, debris, or water bottles are to be left on site. No profane or abusive language. No fighting.  You are responsible for your group and any replacement / repair costs for damage caused by your group’s use as determined by ECEC. No open fires or charcoal grills allowed. Gas grills must be approved in advance, in writing. ECEC reserves the right to revoke or change times and dates on any permit should unforeseen circumstances require it. ECEC reserves the right to cancel this and future permits to any individual/group should the group behave in a manner unacceptable to ECEC and/or violate any of the rules & regulations listed here in.

RELEASE of LIABILITY (READ BEFORE SIGNING):As the responsible representative of the sponsoring group, I am requesting the use of the ECEC Field for the purposes listed above. I agree and fully understand, by signing this waiver, that I must notify the Sponsoring Group, all of its participants and spectators that Elkton Community Education Center assumes no responsibility and no liability for the actions of or injuries to anyone involved with said group through our requested use of this public facility. That responsibility and liability shall fall directly upon the Sponsoring Group and its representative(s). I agree to enforce the rules of the facility on behalf of the Sponsoring Group or to name an appropriate person to do so.  Should I be unable to enforce these rules, I agree to immediately call ECEC POC to assist!  Any damage/ vandalism done as a result of our use will be our responsibility. We agree to pay any reasonable costs associated with this damage. I, the sponsoring party waive, release and forever discharge the Elkton Community Education Center, its staff, volunteers and Board of Directors, and property owner Carol Beckley, from any and all reimbursement, liability claims, demands, actions and cause of action whatsoever arising from or related to any loss, personal injury, disability or property damage that may be sustained or occur during the participation
