To begin the process of identifying a student who may qualify for gifted and talented services, please complete this form. Please note that if a student is recommended, that does not automatically qualify him or her for gifted services in Washington County Schools.

Please check each area that applies to your recommended student most of the time. This will help determine which assessments will be used to evaluate the student for gifted services. Please note that you do not have to choose boxes in each category.

For example, if you want to recommend a student for identification in math and visual art, you may skip all other sections.
