Proof of COVID 19 Vaccination for CTE Industry Fair

On August 18, 2021, Governor Inslee announced a directive requiring all employees, contractors, volunteers, working for public and private K–12 schools to be vaccinated against COVID-19 by October 18, 2021.
I attest that I am presenting Battle Ground Public Schools with access to my COVID-19 vaccination card to verify my status.

I also agree to follow all guidelines set forth by Governor Inslee and the OSPI guidance that school districts are required to adhere to while on any property owned or leased by Battle Ground Public Schools.

Once you have registered, we will contact you and set up a time to complete the Proof of COVID 19 Vaccination verification form.

As guidelines may change, we will keep you informed.
