Fun Fore All Lock-In UYM Parental Consent Form
We are headed to Fun Fore All in Cranberry, PA for a night of games, go-karts, bumper boats, miniature golf, and pizza. Cost is $40/student.

We will meet in the parking lot of Union at 8pm on Friday, May 31. Pick up will be 6am on Saturday, June 1.

From the church we will go to Hillside Christian Community (1050 Campbells Run Rd, 15106) to play some games and hangout. At 11pm will take a bus from Hillside to Fun Fore All. We will be at Fun Fore All from 12 - 5am. At 5am we will take the bus back to Hillside and then we will come back to Union for pick up at 6am.

This consent form gives permission to seek whatever medical attention is deemed necessary, and releases Union Presbyterian Church of any liability against personal losses of the above named student. I the undersigned have legal custody of the student named below, a minor, and have given our consent for him/her/them to attend the Fun Fore All Lock-In. I understand the inherent risks involved in related activities and hereby release the Church, its pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my student's/ students' involvement in this event. *
