Place Matters Information Session Attendance Confirmation
As part of United Way of Forsyth County’s Place Matters 2021-22 Investment process, interested organizations are required to participate in a Place Matters Information Session.  This information session introduces applicants to the Place Matters program, to include the basic tenets of place-based service delivery, resident engagement, DEI (diversity, equity, and inclusion), and Asset Based Community Development.

In lieu of an in-person information session and usual methods for capturing attendance, organizations are instructed to participate via a virtual session and confirm attendance.  By completing this form, organizations attest that they have either participated in the “real-time” information session or viewed the archived version.  Only organizations that have participated in the information session (or watched the archived session)and are recommended by the Place Matters Resident Impact Council will be moved to the application phase of this investment process.
(*Only one individual from the organization needs to complete this attestation.)
