If your child is riding the bus home from EVERY away meet, this does not apply to your family. You do not need to complete this form and may exit the form.

Part 1: Transportation Google Form

You MUST complete this form:

If you wish to transport your son/daughter home from an away competition where the district has provided your son/daughter with transportation.

OR

If you wish for your son/daughter to ride home with another parent from any away meet, you must complete this form.

Submitting the form = LEGAL CONSENT.

Your runner is allowed to ride the bus home even if you submit this form. The runner will let us know that day that they are riding the bus home.

Part 2: Paper signature at away meet

We have a second step for you or the carpooling parent to complete at the actual event:

Each runner not riding the bus home must have a parent/guardian/carpool parent sign a physical sign-out sheet before they leave the event. This is an absolute necessity as well as this google form.

I understand LS middle school policies require student-athletes to ride the bus to all school activities and returning from all out-of-district school activities, and a departure from this requirement will release the LSR7 School District from all liability for any adverse results that may occur. I agree to release the LSR7 School District and its employees and officers from any and all liability with references to all AWAY cross country meets. I understand that there is no bus home for in-district or home meets (including the Longview meet), thus parent/guardians do not need to sign runners out at those in-district/home meets. *
