Just 4 Kicks Infant Football for children in Year 1 & 2 is now full. If you would like your child to be added to the waiting list, please complete this form and we will let you know if a place becomes available.


You should use this form to book a place for your child on the Just 4 Kicks or Fern Hill football club running during the summer term.

Clubs will run for 11 weeks, starting the week of Monday 26th April 2021 and ending the week of Monday 12th July 2021. The week of Monday 19th July is a catch up week when any cancelled sessions will be made up.

All clubs run after school from 3:30 – 4:30 pm, except the club for Year 1 and 2 on Mondays which runs from 3:15 - 4:15 pm.

Please note:

1. The clubs are for children in Year 1 to Year 6 ONLY
2. Payment must be made through ParentPay at the time of booking.
3. The deadline for booking and making payment is 9:00 am on Monday 22nd March.
4. Children must wear shin pads and long socks in order to participate in the football clubs.

Please complete a separate form for each child you would like a book for.

Section 2 asks for information about your child, including parental and emergency contact telephone numbers and medical details. It also asks for information about who is authorised to collect your child from a club session and, if your child is in Year 6, whether they will have your permission to travel home alone.

Section 3 is the Extracurricular Clubs Home School Agreement. Please read it carefully and sign and date the declaration.

Section 4 is the booking form where you can select the club you would like to book.
