This form is completely anonymous, unless you provide your name/alias or specific information regarding the contents of the commission. If you've commissioned multiple items from me (eg Translations and Mix), please fill out a new form for each different item! If you've commissioned the same item from me multiple times, you may fill this out for each commission OR for an average of all the commissions.
