Thank you so much for starting the first step of the process to Coaching. You're already on the road ... where do you want it to take you?

Maybe you know the scene with the Rodgers & Hammerstein song, "Getting to Know You" from the 1956 movie, "The King and I." Please note: this form should be completed at least three business days prior to us meeting.

This form will help me learn what *you* believe *you* need to be successful. It will help me discover what you're looking for in a coaching partner and it will help me determine the best way for us to work together.

So, before you start, I ask the following of you:

1. Please be prepared to set aside uninterrupted time to go on this adventure.
2. Turn off your ringers and notifications while completing this part of our process (even if you're using your mobile device to fill this out) and also when we meet.
3. Set a timer for 23 minutes to complete this form.

You'll answer a few easy questions first (name, contact info) and then you'll be taken to the next step of the adventure. You will automatically receive a copy of your responses to the email you list below, so please choose an email that is confidential and private so that others do not see your responses. We keep all responses confidential and we follow the European model for data-sharing: we will NEVER share your information without express, written permission from you, and any data that might need to be aggregated will be devoid of your personally identifiable information.

** Words to the Wise: Be honest in this process. No one will see this but you and me. If you aren't brutally honest about this part, it will take more work to get you the results you want from working with me. This is very much a "keep your eye on the prize" experience. In case you haven't guessed it, you are your own prize. :D
