DANCER AGREEMENT: Please read through each of the following expectations for participation in this program and check that you are willing to agree to these terms. These will be the contract terms that each dancer will be asked to commit to, in writing, in order to participate in the program. *

I will have a positive attitude at all times when working with my Elite Masterclass Training Series mentor/instructors.

I will commit to working at 100% and striving for excellence in every class

I will put myself in the right training mindset everytime I step foot in the studio, because I deserve it

I will respect my mentors/instructors and peers at all times in and out of the studio

I understand that the mentors/instructors are giving me their full energy and knowledge in order to help me reach my full potential as a dancer

I understand that I wouldn't be here without my parents so I will be respectful and grateful to them

I will be an ambassador for MDM and KM Agency and take pride in being a part of the Elite Masterclass Training Series: as well as the way I represent myself outside of the studio, on social media, and in my personal life

I understand that I will be placed in a class by MDM that is in line with my Fall 2023 jazz class placement.

I understand that this is a program that is optional, but will require a commitment to consitent attendance and hard work.

I understand that this program will carry an extra cost of $83/month from September - May and is not part of the multi-class discount.

I understand that this class has class capacities and that I may not be able to take it based on those capacities.

I understand that this registration IS a commitment to enrollment in the program. It is not merely an "interest" commitment, but a commitment to participate.
