Holland Patent Central School District        Re-opening Survey
The Re-opening plan will serve as a guide to safely reopen our schools and provide a learning environment in which our students, staff and families feel safe.  While educating the students of our community is our top priority, the health and safety of our students, staff and families is also crucial to a productive and positive learning environment. This plan is a fluid document as there will be changes to our protocols and procedures based on updated information we receive from the Department of Health, CDC, NYSED and the Governor's office.  In order to implement this plan we need to collect the following information.

Please complete the survey below for each student who attends the Holland Patent Central School District.  We understand circumstances do change and you may need to modify your responses in the future.  Responses to this survey must be completed no later than Monday, August 10th.

Instruction
The Holland Patent Central School District is operating a hybrid model that will allow all students to receive in-person and remote instruction, allowing students to receive as much in-person, live instruction as possible. Delivering the hybrid model will ensure students receive equitable, high quality standards based instruction which includes daily interaction with teachers in order to meet their academic needs.  Students may also choose to attend school remotely based on individual needs and preference.
