Due to concerns on the current corona virus crisis, only those with confirmed appointment schedule shall be allowed entry to the University of San Agustin campuses.

Fill out the form below.  The confirmation of your appointment schedule will be sent via text message so a valid cell phone number is needed.

Please take note of the following:
1.  Persons with confirmed appointment schedule shall abide by the security, hygiene, and sanitation protocols of the University.  
2.  Present proof of the confirmed schedule and a valid ID at the gate. No ID, no entry.
3.  No face mask, no entry.
4.  Persons with body temperature higher than 37.5 degrees shall be denied entry.
5.  We do not encourage students and those over 60 years of age to come to the campus at this time. Please visit the University website for ways to contact the concerned unit.
