Please complete the form below to register your youth (7th-9th grade) for the Our Whole Lives sexuality education program. Some necessary information will be shared with the trained facilitators, such as allergies and special needs. We will use the information on this form to communicate with you and to ensure we provide a caring, safe environment for your youth and all participants in our OWL program. 

Our OWL program will run on Sundays 3pm-5pm on the following dates:

There will also be a parent orientation  on October 15th, 7pm-9pm. 

If you have multiple children in the program, you can use this form for all of them. There is also space for up to four parents/guardians on this form. Please skip any section that is not applicable to you. We do require parent/guardian information for at least one adult.
