I will not hold Pawsability or Jade Empire Pets and/or their trainers liable for any accident/injury occurring on or arising from the normal course of the program due to any pre-existing condition/issues not disclosed by myself.
I hereby accept and assume, without reservation, all risks associated with my participation in the Classes/Training, including, but not limited to: the risks of any and all injuries to myself, my dog and any of my family members or third parties who may attend from the nature of any seen or unforeseen circumstances; the risks that my dog may cause injury to other persons and/or dogs while training if not monitored attentively
All animals are boarded and/or trained or otherwise handled or cared for by us without liability on our part for loss or damage from disease, death, running away, theft, fire, injury to persons, other dogs, or property by said animal, or other unavoidable causes, due diligence and care having been exercised.
I do authorize emergency first aid and/or veterinary care by Pawsability trainers in the event that I/my dog become/s injured or ill during the program.
I acknowledge that I have read and fully understand the information in this form. Any questions concerning this information have been discussed. CANCELLATION POLICY
Board and Train- Cancellation of program must be made TWO WEEKS in advance to avoid penalty fees                                                                                                                  -If package is cancelled within this time frame, a refund of 80% of package price will be given.                                                                                                                              -No Shows or cancellations made less than 24 hours in advance will be subject to FULL PACKAGE CHARGE.

Classes
All cancellations must be made at least 24 hours in advance of your class to avoid any penalty fees.                                                                                                       Cancellation fees are as follows:                                                                                    
*Private sessions- No Shows or Cancellations made 5 hours or less before classes are subject to FULL CLASS CHARGE.                                                                *Group Sessions - Cancellation of package must be made ONE WEEK in advance to avoid penalty fees.  If cancellation is made within this period, a refund of 80% of package price will be given.

Payments can be made via cash/cheque, or online bank transfers.  
Returned cheques are subject to a $60 charge which must be paid when cheque is replaced.                                                                                                                                                                                                  Payment is due IN FULL prior to classes commencing or on the first day of class.
