AFTER you have signed up with Inspire Diagnostics, if you ONLY want SVUSD to have permission to utilize the PCR testing for EXPOSURE / EVENT TESTING ONLY, please indicate below.
If you want your student to be included in BOTH surveillance and exposure/event testing using PCR testing, please register with Inspire Diagnostics and DO NOT COMPLETE THIS SURVEY.
