Ignite Bristol will bring together people from the disability, sport, health and education sectors to support more disabled people to access inclusive sport and physical activity opportunities throughout life.

This survey is to be completed by disabled people, and where needed with support from their parent/carer. It should be focused on the disabled person’s sport and physical activity experiences.
