Jubilee United Church - Charity 5 km Walk/Run -October 14, 2022 (Rain date – October 15, 2022)
(hereinafter referred to as "JUC -5K")

Thank you for registering to enter the JUC-5K.

In view of the ongoing pandemic, the public health emergency, and the health vulnerable nature of the Jubilee United Church community, the Board of Trustees for the Congregation of Jubilee United Church and the organizers of the JUC-5K strongly urge you to participate in the JUC-5K only if you are fully vaccinated as defined by local health authorities.

This notice confirms that your entry has been accepted subject to your agreement to all of the terms and conditions applicable to participation and subject to your execution of the Release and Waiver of Rights set forth below.

Please be aware that when you participate in the JUC-5K, you do so entirely and solely at your own risk and that you voluntarily accept all risks associated with such participation. By signing this document, you represent and warrant that you are physically fit to participate in the JUC-5K. Further, as a participant in the JUC-5K you hereby agree to follow all applicable COVID-19 protocols and procedures required by local health authorities and in effect as at the date of the JUC-5K.

Finally, by signing below, you agree that none of: Jubilee United Church, the Congregation of Jubilee United Church, the Board of Trustees for the Congregation of Jubilee United Church, the organizers of the JUC-5K, the Employees and volunteers of Jubilee United Church in any way involved in the conduct or the organization of the JUC-5K, (hereinafter collectively referred to as "Releasees") are in any way liable or responsible for any adverse health effects, including, without limitation, illness, injury or death, damages or loss to your property, however occasioned, arising from your participation in the JUC-5K. Further, in consideration for the acceptance of your entry to participate in the JUC-5K, you, on your own behalf and on behalf of your administrators, assigns, estate trustees, executors, heirs and successors, as applicable, do hereby RELEASE, WAIVE AND FOREVER DISCHARGE all Releasee(s), its and their respective administrators, assigns, estate trustees, executors, heirs and successors, as applicable, of and from all related actions and causes of action, claims, costs, damages and, demands whether in equity or in law.
