Please complete this form to register for one of the Duke of Edinburgh Awards and acknowledge the implications and costs of doing so.
There will be separate sign-up form for the expeditions through our normal Evolve Trips system
Charges
A £40 charge will be added to your account, normally at the end of the Michaelmas term.  Please note, if your child decides to cancel their participation after their DofE profile has been created, the DofE registration fee will be charged to your account.
Application Process
The first 72 applications received will be accepted.  A waiting list will be opened for other applicants.
