TEAMM Site Intervention Application
Thank you for your interest in receiving a site intervention from the Training, Education, & Advocacy in Miscarriage Management project. The goal of our project, established in 2008, is to mainstream the education of clinicians and staff to incorporate comprehensive outpatient uterine evacuation into settings where it is not fully integrated into teaching, training, or clinical services. As experts in practice integration, our approach aims to help sites:
               •   Implement necessary protocols, equipment, and medications
               •   Train providers and support staff to offer patient-centered care
               •   Offer training tools necessary for successful integration into curricula

TEAMM is funded by the Washington Department of Health and a private donor.

In order to be considered to receive a training intervention for clinical sites outside Washington State, please complete this questionnaire.

If your clinical site is located in Washington State, please do not fill out this form. Instead, please apply be emailing Erin McCoy, TEAMM Project Manager: teamm@uw.edu.
