Ivy and Oak Farm Floral Preservation Booking Request Form
Please take the time to read through and correctly select the pieces you're interested in booking. We accept flowers from across the USA. This does not guarantee booking, I will email you to confirm your date, and further instructions on how to book with Ivy and Oak. Please also note that the final selection of your pieces will be made once your flowers are done drying. Once you're booked with me, you can add and edit pieces until our final consultation when the final payment will also be due (approx. 1 week before I start working on your pieces).

*To book with Ivy and Oak, please ensure your pieces meet or exceed the $350 minimum.

I will tag them on Instagram, so I like having their information. I will not reach out to your florist. 
Please let me know which pieces you're interested in booking. I encourage my clients to think about how they might want to use / display the pieces they book. Photos and pricing can be found here.

Please Note: you'll be able to add/edit pieces during our final consult when your florals are dried and I'm about a week out from starting on your piece.

Example: 10x10 tray, set of coasters, 7 ornaments, etc. If booking a piece with wood, please also let me know if you're interested in a darker wood or lighter wood. 
I acknowledge that there is a $150 non-refundable booking deposit that will be due to book Ivy and Oak Farm's floral preservation services. This payment is due once both parties agree to move forward with booking and will be applied towards the final balance when it is due. Invoices are sent via PayPal and can be paid with credit card or check. If paying with a card, a 4% CC processing fee will be added.  *
