Appointment Information:
  •  All vaccine costs can be located on our Vaccine Cost List.
  • We accept cash, check, or credit card. If using a credit card, be advised Davis County Health Department charges a 2.50% processing fee with a minimum of $1.55. 

Please arrive 10-15 minutes early to your appointment. 

If you are more than 10 minutes late, you may be asked to reschedule.
