This is the form prepared as the communication tool between the service agent and EYEMETES. This form does not mean that the given work would be undertaken by EYEMATES. The finalization of work approval can be decided only after discussion with the service beneficiary. The remuneration for the work is considered as 10 per cent of the fees received from the Service Beneficiary and the fee structure would be decided during the work finalization process.
