Preschool - 8th Grade Registration Form 2023-2024
For families wishing to enrolling their children in Preschool  - Grade 8

Checklist of items needed for initial registration (PRIOR to acceptance) Links for forms available at the bottom of the registration form:

___ Pastoral Verification Form - (one per family Preschool - Grade 8) 
___ Health Physical (For students entering Kindergarten and 6th grades only - contact Michelle for details)
___ Copies of Sacramental Records (For New JFK Students)
___ Copies of Official Birth Certificate (For New JFK Students)  
___ Proof of immunizations and medical evaluations (For New JFK Students)
___ Enrollment in FACTS Management System - (one account per family Preschool - Grade 8)
___  Registration fee of $100 per family (billed on FACTS) needs to be submitted to the school office and will secure your child's seat for the 2023-2024 school year. This fee will include the FACTS enrollment fee, payment plan fee including incremental billing, and fees associated with the FACTS Student Information System. (*This does not include the $25 application fee for FACTS financial aid if you choose to apply for tuition assistance.)

Financial Obligations: Enrollment for all students at John F. Kennedy Catholic School is contingent on all financial obligations being met.  Failure to satisfy financial obligations may result in the space(s) being reserved for your child(ren) being given to a child(ren) on a waiting list.  

Wait List: Any applicant applying for enrollment in a class that has already been filled for next year will automatically be placed on a wait list and notified if an opening becomes available.
