By completing this form, you understand and agree to the following:

* All Sessions must be paid for in advance.
* Cancellations or Reschedules must be made with at least a 48 hour notice or there will be a $50 reschedule fee assessed.
* Cancellations made the day of the lesson will not be refunded or credited.
* If a player in your group is a "No-Show", the billed rate does not change.
