PERSONAL DATA CONSENT

PHINMA COC recognizes their responsibilities under the Republic Act No. 10173, also known as the Data Privacy Act of 2012, with respect to the data they collect, record, organize, update or modify, use, share, retain, retrieve, consult, consolidate, block, erase or destruct from students, employees, alumni, affiliates, contractors, visitors, partners, and vendors. The personal data obtained from the data subject or an individual is entered and stored within the company authorized information and communication systems and will only be accessed by the authorized personnel. University of Iloilo has instituted appropriate organizational, technical and physical security measures to ensure the protection of the collected personal data. Furthermore, the information collected and stored shall be used for PHINMAED SHS SCHOLARSHIP APPLICATION, and, will be uploaded in PHINMA COC's MASTER DATABASE.
