The purpose of this form is to create your user account in Google Cloud - G Suite and we will use this Section 2 (Communication Details) for communication in the Parish G Suite Apps (Gmail, Calendar, Currents, Hangouts, Drive, Docs, Sheets, Slides, Keep, Forms, Sites, Jamboard, App Maker and Vault) and WhatsApp.

Google Cloud - G Suites Applications allows you to access and use our parish email domain "@csfaministries.sg" for ministry/group or "@csfa.sg" office for staff. All parish internal and external communication must use parish email for official use.

In compliance with the Personal Data Protection Act (“PDPA”), The Church of St. Francis of Assisi (“SFA”) seeks your consent to collect and use your personal data (i.e. Name, mobile/WhatsApp numbers, mailing and email addresses) in order to maintain the Google Cloud G Suite registry of membership and to disclose such personal data to Church SFA affiliated organisations where necessary (“Purposes”).

IMPORTANT: In compliance with guidelines of the Personal Data Protection Act (PDPA 2012) and our Policy https://www.catholic.sg/pdpa-policy/ 

In filling this form, I consent to:

(a)  SFA Parish Office and SFA Ministry/Group for the collection, use and disclosure of my personal data to provide information on membership and activities such as upcoming events, seminars, workshops and training programmes organised by SFA Parish Ministry/Group which may be relevant to me (“Services”).
(b)  The collection, use, disclosure, storage, retention, adaption, modification, reading, retrieval, transmission, blocking,  erasure or destruction (“Processing”) of the personal data provided by me in this form (“Personal Data”);
(c)  The church entity processing my Personal Data for the purpose of creating my user account in Google Cloud - G Suite and use for communication in Church of St. Francis of Assisi parish.
(d)  The church and its affiliates to take and store photographs and video footage to be used in the promotion of our activities in our community and the Catholic Churches.

- I hereby give my acknowledgement and consent to the Parish Office and SFA Ministry/Group to use my personal data for the aforesaid Purposes and Services. In the event that I have registered my Singapore telephone numbers(s) with the Do Not Call Registry and wish to withhold or withdraw my consent to Parish Office and SFA Ministry/Group. In respect of receiving telephone calls and/or SMS, I endeavour to provide sufficient notice to Parish Office and SFA Ministry/Group of such as soon as reasonably practicable. I further agree to indemnify Parish Office and SFA Ministry/Group against any financial penalties imposed by the Personal Data Protection Commission or any court of law in Singapore as a direct or indirect result of my failure to inform Parish Office and SFA Ministry/Group of my registration with the Do Not Call Registry.

- I agree that my consent will remain in place until my withdrawal by officially notifying the Parish Office and SFA Ministry/Group in writing at dpo@csfa.sg
