Consent of Participation
I acknowledge that there are risks in any program. I declare that I have accurately disclosed all information regarding my daughter/son's physical, mental, or medical conditions. I under stand that ProBots Ottawa has tried to create a safe and controlled environment for participation.  I waive the rights of my daughter/son to damages or other costs in the event an injury is caused due to participation here.

Even though the Government of Ontario has removed the mask mandate, we recommend a mask to be worn during participation in VRC Robotics Competitions.  We also support receiving your vaccinations.  

I acknowledge that in order to participate in the VRC Robotics Competitions there will be travel and tournament registration costs involved as most tournaments are in the GTA area.  I will also be willing to volunteer to help with tournaments in the Ottawa area.
