COSTA RICA Trip Sign-up
All students and parents attending the trip need to turn in a SEPARATE FORM.  This means that if mom and dad both plan to attend, I must have a form from each attending person.  In addition, each person attending will need to fill out THIS google form.  Only those I have a signed form, deposit, and google form with all matching information will be added to the trip roster.  A list of those attending will be put on the band app to double check that no one was missed in the process.
