Do you have any health conditions? If yes, please describe the condition, along with any knock on effects to training, body composition or food cravings we should be aware of.  *
How often do you typically train? Include CrossFit Classes, Open Gym and any other sports or activities you pursue outside the box. Give us a sense of the volume of training you do each week, and if it varies at all. Explain why, if so.  *

Have you ever had an eating disorder? *
If you've had, or do have disordered eating, please share anything you feel we should know to effectively coach you.

What do you struggle with when it comes to your nutrition? Do you feel you snack too much, do you have a real sweet tooth, do you struggle to eat enough protein, is it a late night craving, not knowing how to fuel your training?

What is your weekly alcohol intake on average? What is your relationship with alcohol? Your favourite tipple? When do you usually consume? Is it a habit or a special occasion beverage? *
How would you describe your energy levels throughout the day? Do you bounce of bed, do you get a mid morning / mid afternoon slump, do you fall asleep on the sofa by 8pm? *
How would you describe your sleep? Like a baby every night? Broken? Hard to get to sleep? Wake up 2-3 times a night for a pee... ?
