Milton Avenue School Out-of-State Travel/Quarantine Notification Form
Please complete this form if your Milton Avenue School student will be traveling to any state outside of NJ, NY,  CT, DE and PA over Spring break. Indicate the date on which your child will return to New Jersey. Day 1 of the required 10-day quarantine will begin the day after return. The student may return on the earlier indicated date with proof of negative COVID test (PCR only) taken on Days 3-5 after return to NJ. Your child may only return once the negative COVID test is emailed to the school nurse at ezotti@chatham-nj.org.One form should be submitted for each child and submitted by March 5th, 2021. Required Quarantine after Travel

As per the NJ DOH, all non-essential interstate travel is strongly discouraged at this time.  

If a student or staff member has traveled beyond the immediate region (NY, CT, PA, DE), they must quarantine.

Travelers should quarantine for 10 days after travel.
