Please complete this form to signup to attend the sport physicals being held by the Oley Valley School District on Monday, August 3rd from 6PM-9PM. The cost for the sport physical is $10, paid at the time of the physical. Checks should be made payable to "OVSD". Appointment times and more directions will be emailed to the email address listed on this form. The appointment times and directions email will be sent the week of July 27th. Registration for the sport physicals must be completed by Sunday, July 26th. While we will try to schedule all student athletes, the number of appointment times is limited so Fall student athletes will be given priority.
