Following city, state, and diocesan guidelines, St. Peter's is allowing small ministry groups to gather for outdoor, in-person gatherings. In order to uphold the dignity of every human being and live out our Christ-like love for neighbor, there are safety guidelines all participants must follow when gathering. All participants are required to complete this form PRIOR to the event. By submitting this form, you are agreeing to follow these guidelines. This form will also be used to responsibly contact trace in the event that someone who attends tests positive for COVID.

Please select all guidelines. By checking each box, you agree to comply with these safety guidelines for small group gatherings in person. All boxes must be checked in order to attend this gathering. *

Masks are not required outdoors for those who have been vaccinated. If you have not been vaccinated, we strongly encourage you to wear a mask.

Maintain distance from those not in your household or pod.

Anyone who does not feel well or who has been exposed to covid in the last 10 days should not attend.

Food may not be shared- people need to bring their food/beverages/utensils with them and remove their trash at the end of the gathering.

Families with children are permitted, but parents/guardians must be present.

Ensure you have all of the supplies and equipment you need for a successful/fun gathering.

If I find out that I was exposed to COVID in the 10 days prior to the event, I will contact Kate Randall randall@stpetersphila.org.

If you are the leader/coordinator for this gathering, you must also agree to the following:

Attendance must be taken and the list sent to Kate Randall.

Someone in the group needs to make arrangements *beforehand* to get keys to 313 Pine for bathroom accessibility.
