The Sacrament of Confirmation Questionnaire
We are so excited to journey with you towards the sacrament of Confirmation.  In order to best prepare for our next steps, this questionnaire will help us plan, prepare and communicate in the coming year.

Please Note -  We must have a copy of your child's baptismal certificate on file. If your child was Baptized at Holy Trinity, we have that record on file. If your child was Baptized outside of Holy Trinity, please call that parish and request a certificate be sent to Holy Trinity attention Pat Terry. If you have any questions regarding this step, please contact Pat.
