I hereby waive all claims against the Cocoa Beach Uncorked, Floridata Capital Assets Group, Inc, the City of Cocoa Beach, promoters, organizers, producers, sponsors, event staff, and any personnel or company affiliated with Cocoa Beach Uncorked for injury, accidents or physical conditions I may suffer at this event.  

I grant full permission for organizers to use my name, likeness of voice and photographs, videotapes, or quotations from me in accounts, promotions and activities, in any medium, related to the Cocoa Beach Uncorked event.

I also understand that Volunteers/Staff will not drink during his/her shift times or misrepresent the event or its sponsors.

I certify that I am at least 21 years of age or older.
