OCS Summer Camp 2021 Sign-Up
This form is non athletic camp sign-up.  Please use 1 form per camper. Approximately, 2 weeks after sign up, FACTS will send the bill for payment.   Non OCS campers are welcome and can send a check to OCS within 2 weeks of sign up.  3075 Shimmons Rd, Auburn Hills, MI 48326.  If you sign up for a camp with a field trip, we will get you a field trip permission slip to fill out.   Questions?  Please email Mrs. Prantera at jprantera@oaklandchristian.com. Do you agree to this Summer Camp waiver?                 I, the parent, give my permission for my child to participate in the OCS Summer camp and its activities. Should a medical emergency arise, Oakland Christian School, or its agent, has my permission to seek and authorize emergency medical care for the child in my absence. I agree to indemnify, hold harmless, and defend Oakland Christian School, its officers, directors, shareholders, employees, agents, volunteers, successors, and other affiliates (collectively the “Indemnified Party”), against any and all losses, damages, liabilities, deficiencies, claims, actions, judgements, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including reasonable attorney fees, resulting from or arising out of any claim alleging (1) breach of any representation or warranty in connection with this activity or field trip; (2) any act or omission of the Indemnified Party in connection with this activity or field trip; and (3) any bodily injury, death, or damage to real or tangible personal property arising out of or in connection with this activity or field trip.
