The EOS Mainnet will be performing an important upgrade on early September,  to version 1.8, that will affect consensus (hard-fork). This form is designed to collect information about the current state of the dapps running on the EOS mainnet. BPs can use this information to better plan the procedure and make it as smooth as possible for all users.
