Assumption of Risk: I understand that my volunteer activities may involve inherent risks and hazards, both known and unknown. These risks may include, but are not limited to, physical injury, property damage, illness, and emotional distress. I voluntarily assume all such risks associated with my participation in the volunteer activities organized by the Organization.

Health and Safety: I acknowledge that it is my responsibility to assess and ensure my physical and mental fitness to participate in the volunteer activities. I will take appropriate precautions and follow any safety guidelines or instructions provided by the Organization. I agree to promptly inform the Organization of any changes in my health or ability to participate in the volunteer activities.

Code of Conduct: I agree to abide by the Organization's code of conduct and behave in a respectful and responsible manner. I will treat fellow volunteers, staff, and individuals served by the Organization with dignity and respect. I understand that any violation of the code of conduct may result in termination of my volunteer involvement with the Organization.

Confidentiality: I understand and acknowledge that during my volunteer activities, I may have access to confidential and sensitive information related to the Organization, its clients, or stakeholders. I agree to maintain the confidentiality of such information and not disclose it to any unauthorized individuals or third parties, both during and after my volunteer engagement.

Intellectual Property: I acknowledge that any intellectual property, including but not limited to trademarks, logos, copyrighted materials, or proprietary information, associated with the Organization, shall remain the exclusive property of the Organization. I agree not to use, reproduce, or distribute such materials without the prior written consent of the Organization.

Release and Waiver of Liability: In consideration of being permitted to participate in the volunteer activities, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless the Organization, its officers, directors, employees, and volunteers from any and all claims, liabilities, demands, actions, or causes of action, whether arising from negligence, tort, contract, or any other legal theory, for any personal injury, property damage, or other loss, arising out of or in connection with my participation in the volunteer activities.

Effective Date: I understand that the effective date of this Volunteer Waiver and Disclosure is the date signed and submitted by me.
