Chromebook Return
Please complete this form if you need to keep the Chromebook you checked out from the school for additional time over the summer.  This would be to continue learning or working on any incomplete grades a student may have.  If this form is not completed, we will expect that the Chromebook will returned during your specified time the week of May 18-22.

If you choose to complete this form, and request to keep your Chromebook over the summer, the Chromebook will need to be returned to Matheson Junior High when school begins after the summer, before the first day of school for the 2020-2021 school year.

Students who arrange to keep Chromebooks who will be attending a different school in the fall, will still be asked to return the Chromebook to Matheson Junior High.  If a student is leaving Granite School District, they must return the Chromebook to Matheson Junior High.
