Despite careful and proper preparation, instruction and equipment, there is a still a risk of injury when participating in any recreational activity. Participants must understand that certain risks and injuries due to acts of God, inclement weather, slipping, falling, and all other circumstances inherent to recreational activities/programs exist.

By checking the box below and by agreeing to participate in the program, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which the participant may sustain as a result of participating in any and all activities associated with this program. I desire to have my child participate in all activities, and assume all risk of any and all injuries, damages or loss they may sustain as a result of said participation. I agree to waive and relinquish all claims I may have as a result of my child participating in this program against 613 University/ Caleb Hugh Basketball, including its officials, agents, volunteers and employees.
