Melody and Harmony Registration 
Welcome to Melody and Harmony! 
The following form is the official semester registration for all Melody and Harmony classes. Please read through everything carefully. 
Payment for each class is due before class or upon arrival.

Class Times: 
Saturdays at 9:00am (ages 0-3) 
Saturdays at 10:00am (ages 0-3) 
Saturdays at 11:15am - OPTIONAL Age 4-6 Class if there is interest and registrations 

Schedule: 
The Summer 2025 semester will run on Saturdays from May 3rd through July 26th. There will be NO classes on May 24th and July 5th for Holidays.

Tuition and Payment 
Families will pay at the door for each class. A Music Class Punch Card is available for families who think they can make 8 of the 11 classes and pay ahead of time. 
 
0-3 Year Old Classes 
  • $15 for class when you drop in and pay per class 
  • Punch Card: $115 for 8 classes; paid on the first day of class 

4-6 Year Old Classes 
  • $16.50 for class when you drop in and pay per class 
  • Punch Card: $125 for 8 classes; paid on the first day of class 

Cash and Check are accepted. Melody and Harmony also has a Paypal account; this is the preferred payment method.

Attendance 
Please check any and all dates you plan to attend Music Class. If you are purchasing a Punch Card, you may select the last option and attend any 8 classes throughout the semester. 
I give Melody and Harmony permission to use any photos and/or videos taken during class for social media and promotional purposes.  *
General Liability: As the parent or guardian of the child(ren) in Melody and Harmony classes, I agree that any injury I or my child(ren) incur during class is my responsibility.  *
Please sign and date below to indicate understanding of the following: 
-Attendance for Parents/Guardians is mandatory for 0-3 age classes. 
-Attendance for Parents/Guardians is not mandatory, but HIGHLY RECCOMMENDED for 4-6 age classes. 

The responsibility for each child falls on the shoulders of the parents/guardians present. When parents are not present, any incident or injury incurred during class is not the responsibility of the teacher.
