Due to the ongoing current situation with Covid-19, Kane School District is conducting pre-registration for Kindergarten through the following form.  This is to promote social distancing and keeping school personnel and families safe.  In order to complete student registration, legal documents such as birth certificate, immunization records,  language and special programs will be required to be provided to the school.

These immunizations are required for enrollment for Kindergarten.  Please ensure that your child will  have these completed before registering for school:

5 DTP/DTaP/DT (4 doses if 4th dose was given on/after the 4th birthday)
4 Polio (3 doses if 3rd dose was given on/after the 4th birthday)
2 Measles, Mumps, Rubella (MMR)
3 Hepatitis B.
2 Hepatitis A.
2 Varicella (chicken pox)
