Community Health Needs Survey
You are invited to participate in a research project being conducted by Raghavee Neupane with Dr. Hay-Rollins from the Anthropology Department at Miami University. The overall purpose of this study is to gather information about the health needs of the permanent residents of Oxford, Ohio, and the extent to which the local healthcare currently meets those needs.

If you are 18 years old or older and have a permanent address within the city limits of Oxford, Ohio, we really want to hear from you!

Participation consists of completing an online survey. The survey takes 5-10 minutes and you can access the survey through any electronic device with internet connection.

Your participation is voluntary: you may skip questions you do not want to answer and you may stop at any time. Only the research team will have access to individual responses. The survey does not ask your name, address, or any other identifiers that could connect you with your responses.

Online activity is subject to hacking. You should be aware of your surroundings when completing this survey to ensure your responses cannot be observed by the unintended. Once data has been downloaded and archived locally, the responses will be deleted from the google server to the best of our ability.
