Class of 2021- Please use this form to request transcripts for the 2020-2021 school year. You may refill this to add additional transcript requests, however- please note:

**IF YOU ARE UNDER 18, a parent must digitally sign this form
**ONLY THE FIRST THREE TRANSCRIPTS ARE FREE- after the first three, $1/per request must be paid to the CHS Guidance Office (if you are fulltime online, you may mail it or drop it by the front office)
