By signing these documents, you are agreeing to follow all the guidelines BBSC are putting in place and understanding regardless of the presence of these guidelines that risk may still be present. By signing this document, you are accepting these risks and accepting full responsibility of your own health. You are also agreeing to keep clear of the sailing club if you are feeling unwell, to prevent putting yourself and/or other members at risk. If any of the above changes it is your duty to inform the club and seek medical advice.

1 Do you feel fit and well enough to partake in sailing activity’s that may be either on the water or within the club grounds? *
2 On entrance to the Sailing club do you agree to keep 2 meter distances until government restriction lessen and only enter the club if necessary along with using hand sanitizer on entry . *
3 Do you agree to follow all rules and guidelines that Bantry Sailing Club clg, HSE, HSA and government have all put in place? *
4 Do you understand and know that using the sailing clubs facilities is your own prerogative and your responsibility as a member? *
5 There will be club sailing and club racing that may take place and in that case are you comfortable with groups of people? *
6 Do you understand that the club is taking into account all risks, but it is your responsibility to wash hands and use the hand sanitizer supplied? *
7 Have you read through the Response plan for Covid-19, and accept actions are in fact being taken into consideration? *
8 We are advising that in big groups (15+ people) that the use of masks are beneficial and a recommended precaution. Are you comfortable wearing and using a masks? *
9 Are you comfortable with the protocols that are being put in place by Bantry Bay Sailing Club to help prevent further spread? *

Please confirm you will not participate if you have the answer of yes to any of the following ...
a I will not participate in club activities if I have symptoms of cough, fever, high temperature, sore throat, runny nose, breathlessness or flu like symptoms now or in the past 14 days? *
b I will not participate in club activities if I have been diagnosed with confirmed or suspected COVID-19 infection in the last 14 days? *
c I will not participate in club activities if I have been a close contact of a person who is a confirmed or suspected case of COVID-19 in the past 14 days (i.e. less than 2m for more than 15 minutes accumulative in 1 day)? *
d I will not participate in club activities if I have been advised by a doctor to self-isolate at this time? *
I agree to inform Bantry Bay Sailing Club clg if I am diagnosed with Covid-19 and have been in the club environment within the previous 10 days. *
