I, The PARENT OR LEGAL GUARDIAN authorize permission for the above listed person, perform as a dancer in the The Tales of Beatrix Potter” rehearsals and performances from January 28th, 2023 to May 20th, 2023 to take place at Alamo City Arts Academy (ACAA) located at 2800 NE Loop 410, San Antonio, TX 78218, the McAllister Fine Arts Building located at 1300 San Pedro Avenue on the campus of San Antonio College and Fort Sam Theater located at 2472 Stanley Road, 78234 on JBSA.

I release the Alamo City Performing Arts Association, Inc. (ALAMO CITY ARTS), Alamo City Dance Company (ACDC), and Alamo City Symphony Viva and its directors and Association Officers from all claims and liabilities in the event of injury and will hold neither the Alamo City Performing Arts Association, Inc., Alamo City Dance Company, Alamo City Symphony Viva its directors and Association officers or its agents responsible for possible accident or injury. In case of medical emergency, I authorize the Alamo City Performing Arts Association, Inc. and or its agents to transport my child to a medical facility for medical treatment. Additionally, I release Alamo City Arts Academy, its owners, agents, and employees from any and all claims for accident or injury which could occur or liabilities during rehearsals at the school.

In addition, I understand that all photographs taken of the production in rehearsal or performance are the property of the Alamo City Performing Arts Association, Inc., and the Alamo City Dance Company. I release the use of any and all pictures taken to be used solely for the purpose of advertising the Alamo City Dance Co.’s production of The Tales of Beatrix Potter as ALAMO CITY ARTS and / or as ACDC chooses to reproduce in any media.
