Safe Internet Use
We make use of the internet for some of the teaching in school.  All children will have access in some lessons to the internet.  We have secure filtering systems and regularly review the content pupils are accessing.  We expect parents and carers to do the same at home to keep children safe online.  
I agree to keep my child safe when they are using devices connected to the internet.  I understand that not doing this could lead to a safeguarding concern being raised by the school.   *
Local Visits Permission
We make use of our local area to enrich our curriculum and need your permission to take pupils out of school on planned visits.  We have a separate form for parents to complete for educational visits which are beyond 1 mile. 
I give consent for my child to take part in all local off-site activities within 1 mile of the school.   

I acknowledge the need for my child to behave responsibly and in line with school policy whilst they are away from the school site.

I agree to my child receiving medication as considered necessary by the medical authorities present if I am unable to be contacted in an emergency. I understand the extent and limitations of the insurance cover provided.

I will inform the school if my location/contact details for the duration of the visit are different from those currently held.

I understand that should this change, I will notify the main school office in writing to confirm my consent is no longer given.

Medication and treatment permissions for both at school and when undertaking school activities away from the school.

For all trips and off-site activities, it is your responsibility as the parent or guardian, to ensure the following: 

It is vital that you inform the trip leader of any medical condition that your child has.

Failure to do so may result in invalidating the insurance and you would then be liable for the costs incurred in the event of a claim.

The trip leader must be informed directly of any temporary medication that your child requires. This must be in its original packet with full instructions.

I confirm I give permission for my child to receive First Aid treatment at the discretion of the appropriately trained school staff.

I agree to not send my child into school with any food items containing nuts.  This includes food in packed lunches and morning snacks.  This will protect children with nut allergies.  *
