Please place your name on this form if you plan on attending Field Day. If you are visiting with multiple students, you only need to type one child's name and teacher. If you have received a badge previously at Middlesex or any of the Cumberland Valley schools, our staff will use this form to pre-print a badge for you to pick-up in the lobby on Field Day. If you have not been provided a badge prior to the event, you will need to enter the office with a valid driver's license in order to receive a badge and attend Field Day.
