As a parent or carer, you should fill in this form if you need to take your child out of school during term time for reasons other than medical or educational appointments. Please complete this form no less than 3 weeks before the date when you want the period of absence to start. You will receive an email confirmation from the school within 1 week with a decision. Please note, the headteacher may not grant any leave of absence during term time unless it is an exceptional circumstance, this is due to amendments made to the 2006 Pupil Registration Regulations.
