Thanks for helping us spread the word about CTC's Walden Theatre Conservatory!
Please follow the prompts below to enter your contact info and the names of the students you've referred to our programs. You're welcome to return to the form and add more names as you make your referrals.

Each *NEW* student (age 5-18) you refer now through December 31, 2019 who enrolls in the Conservatory for Spring 2019 makes you are eligible for a $50 Amazon gift card. A new student is someone who has not participated in the Walden Theatre Conservatory previously (Conservatory includes Summer Programs).

This tracking form helps us identify who referred the new student, and (in the case of multiple referrals) who we can identify as the FIRST person to enter the referral on this form.

Referrer must be age 16+. For those younger than that, a parent/guardian must complete the form to be eligible.

We are hosting a free info/welcome session for new and prospective students and their families on January 6. Referrals are welcome to visit us online and RSVP for that event before signing up.
