I, the undersigned, hereby authorize Empowering Pacific Islander Communities (EPIC) to use, reproduce, and/or publish photographs, video, and/or audio that may pertain to me including my image, likeness, and/or voice without compensation. I understand that this material may be published or used in various publications, materials, and endeavors related to EPIC's activities, including use on the EPIC website, printed material and social media. This authorization is continuous and may only be withdrawn with rescission of this authorization. Consequently, EPIC may publish materials, use my name, photograph, and/or make reference to me in any manner that EPIC deems appropriate.
